ChartSpan is our partner and helps to provide our "Eligible Medicare Patients" access to a team of Medical Professionals at any time day or night. ChartSpan was developed by the Centers of Medicare/Medicaid in an attempt to provide patients with additional support in keeping patients healthy and from seeking care from a more expensive level of care such as an emergency room.
Depending on your insurance, you MIGHT have a patient responsibility of up to $17 per month for the service; however, most patients will qualify for much less if not ZERO cost. You DO have the option to opt in or opt out of the service as there is NO OBLIGATION.
What is Chronic Care Management? (CCM)

CCM is a Medicare program designed for ELIGIBLE Medicare beneficiaries

The primary focus of CCM is to offer patient-specific support through a quality-driven healthcare approach to patients with two or more chronic conditions.

Eligible patients need to give consent to enroll in the CCM program.
