Position Summary
The Social Worker’s main role is to plan, organize and implement social services and discharge planning for patients and families in conjunction with Case Management.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
- Meet with patients to assess their needs, challenges, and resources,
- Develop treatment plans or support strategies tailored to individual or family needs.
- Provide counseling and emotional support to patients dealing with issues like abuse, addiction, or mental health concerns.
- Connect patients with necessary resources, such as healthcare, housing, food, education, or legal services.
- Help clients navigate complex systems such as healthcare, legal systems, and social services.
- Keep accurate records of patient interactions, case progress, and services provided
- Work with other professionals, such as doctors, lawyers, and educators, to provide comprehensive care.
- Participate in interdisciplinary team meetings to review and adjust patient care plans.
- Educate patients about available resources, rights, and responsibilities.
- Assist patients in hospitals or clinics, addressing emotional, financial, and social challenges related to health conditions.
Minimum Qualifications (Knowledge, Skills, and Abilities)
- Bachelor’s or Master’s in Social Work (BSW or MSW) or Licensed Psychologist
- Current KS License
- Minimum of 2 years related experience and/or training in a similar setting preferred
- Skills- Empathy, active listening, problem-solving, organizational skills, crisis management, cultural competence.
