Community Membership and Group Class Payment Agreement

1. Monthly dues payable by member, effective (date) __________ are $30 (for over age 60) and $40 (for under age 60) and are due on the 20th of each month. A $5 monthly discount will be applied for all memberships set up for automatic renewal. There is a One-time fee of $25 for screening, orientation. Class fees are due 14 days prior to the start date of the specific class.

2. Changes in Monthly Fees: Labette Health reserves the right to change the amount of the monthly membership fee by giving me thirty (30) days written notice of the change. Failure on my part to advise Labette Health in writing of my disagreement with the change within fifteen (15) days of the receipt of this notice by me shall be deemed to be full acceptance of such change. If I give written notice of my disagreement with the changes as provided, this agreement shall terminate thirty (30) days from the date of the original notice of change to me by Labette Health. Any notice to me shall be mailed to the address provided by me unless I give Labette Health notice in writing of a new address.

3. Payment Process: Payment Process: If method of payment is by pre-authorized bank withdrawal (Electronic Funds Transfer), or credit card withdrawal, your initial start-up fees and first month membership fee, or requested amount of month membership fees (1 month, 3 months, 6 months, or 12 months) will be immediately withdrawn from your bank or credit card account. After that fees will be withdrawn from your account monthly, on the date requested above. Payment is required one month in advance for all memberships. No cash will be accepted.

4. In the event of debit card or credit card non payment (bad debt), the member will have five (5) days to reconcile payment. Should the member fail to reconcile, membership will be terminated by Labette Health.

5. Group Class Fees: Any fees for Group Classes may be paid for in person with debit or credit card only. No cash will be accepted.

6. Terms and Automatic Renewal: All withdrawals will be automatically renewed on a monthly basis, unless otherwise agreed upon.

7. Termination of Membership: In order to terminate this agreement, the member must deliver written notice to Labette Health at least thirty (30) days prior to the end of the membership term. Such termination shall become effective on the expiration of the (then) current period. Memberships delinquent by more than 60 days will be terminated. Renewal of terminated memberships will necessitate a new orientation/ fee. Memberships may be placed on hold for up to 60 days with written authorization.

8. Policies and Procedures: The member agrees to abide by the Policies and Procedures of Labette Health Rehabilitation Services for The Center of Rehabilitation Excellence, a.k.a., The CORE, and any amendments, which may be enacted hereinafter. Any breach of these rules could constitute reasons for termination of membership.
